Soyuz Ms-10 Failure

I was tracking the Soyuz MS-10 lift off that had one American and one Russia bound for the space station. The mission had to be aborted because of booster failure. Yeah it was uneventful but what I'm wondering is why are these things still failing? We've been doing space technology for so long and we can't get it right. Maybe there's a conspiracy to all this.
