It depends on the gm, I consider a nat 20 an automatic critical hit. Which means you roll damage and then multiply that by 2 or 3 (says on the weapon description), then add your damage bonus.
So if you swinging a bastard sword and get a 19 or 20 on your roll you then roll 1d10. Lets say I rolled an 8, I then multiply that by 2 getting 16 and then add my damage bonus.
Some GMs have the player roll a secondary d20 to see if the critical strike is in fact a critical hit. Meaning once you have rolled the nat 20 you then again roll another d20. The secondary d20 needs to get past the targets armor to hit and be an actual critical hit. I don't play it that way and I am pretty sure KNtoran does not either.
Out of character: Nope, checking the rules, heal is used to save dying characters and a few other things. Refer to the rules available at the last dot point on the right-hand column above the list of threads when you go back a page.
